Background and History
Lowcountry Male’s mission is to provide a need for more specific and accessible men’s healthcare to the Southeast. It started in the Charleston, South Carolina area (the “Lowcountry”) and slowly expanded to include nine clinics operating in nine different states.
The clinic also ranks in the top 1% of men’s clinics in its main markets and is a “Best of Mt. Pleasant” Men’s Clinic for 2022 and 2023, with high scores of patient satisfaction.

Lowcountry Male operates as a full-service men’s health clinic organized across six service pillars:
Medical Weight Loss:
- GLP-1 Weight Loss Management — semaglutide (Wegovy®, Ozempic®) and tirzepatide (Zepbound™, Mounjaro®); patients lose on average 15% of body weight; lowers A1C and blood pressure
- Phentermine — prescription appetite suppressant
- Fat Burning Shots (B12) — lipotropic / B12 injection support
Testosterone Replacement Therapy (TRT):
- Offers different treatment options including Testosterone injection (cypionate) and oral medications (Kyzatrex®), fertility preservation options (enclomiphene or Clomid) and pellet therapy (administered multiple times per year). A majority of plans also offer assistance with estrogen management, if required.
Erectile Dysfunction:
- Oral medications (sildenafil, tadalafil) as well as more advanced treatments include compounded troches, Trimix/Quadmix injections, shockwave therapy and P-Shot with PRP and performance recovery treatments.
Anti-Aging & Longevity:
- This will consist of IV therapy, NAD+ therapy, metformin, rapamycin, methylene blue, sermorelin, and PRP joint treatments. These are formulated to support long term well-being, cellular health and energy promotion. For continuous monitoring, full lab testing is also available.
Hair Restoration:
- Provides PRP hair restoration, stem cell therapy (DE|RIVE), topical hair regrowth sprays, supplements (Nutrafol) and complete hair restoration programs for hair thinning or loss.
Aesthetics:
- Includes Laser Hair Removal, Medical Grade Skin Care Products (ZO Skin Health), Botox, Xeomin, Dermal Fillers (Juvederm) and HydraFacials.
Peptides:
- Provides healing and performance peptides, such as sermorelin and other personalized peptide protocols based on the individual patient’s objectives.
Doctors and Medical Professionals
Doctors and clinical staff are licensed for all clinics. Michael Twilley serves as CEO/Owner of the practice. Individual doctors’ names are not made public, and the medical team is in charge, from initial consultation to long term follow-up care.
There are four steps to the care process:
- Initial consultation occurs at site with a physical exam, health history, InBody metabolic scan and lab testing.
- Treatment plan is made based on the results.
- Afterwards, medications and therapies are given, often also at home.
- Then, patients are followed up on with regular check-ups and follow-up labs to check for progress and make necessary adjustments.

In 2023, Lowcountry Male won the “Best of Mt. Pleasant” Men’s Clinic award and in 2024, the award was given again. Feedback from patients is generally positive about convenience, regular follow-up and the ease of the subscription model, across the locations.
